Cause

在通过 CC Switch 本地代理向 Xiaomi MiMo 发送带 tools 的流式 /v1/responses 请求时，response.output_item.added 事件中的 function_call.call_id 和 name 正确，但后续 response.output_item.done 及 response.completed 会将两项变为空字符串。Codex 从 done/completed 事件组装工具调用，因此会拒绝这些调用。

该问题报告发生在 CC Switch 3.20.4 的 Windows 11 环境。相同请求使用 stream: false 时字段完整，表明字段丢失发生在流式 SSE 聚合路径。该 issue 仍为 open，尚无已确认的修复方案。

   将问题反馈给 CC Switch 维护者，并提供流式 SSE 捕获；报告建议在生成 output_item.done 和 response.completed 时，沿用 response.output_item.added 中已捕获的 call_id 与 name。

   如调用方能够控制请求模式，可验证 stream: false 是否可避免该问题；但报告指出真实 Codex 会使用流式请求。
