Cause

In routing mode, cc-switch sets ANTHROPIC_API_KEY to PROXY_MANAGED. If this placeholder key is on Claude Code's rejected approval list, it can log out the session and show this error.

The issue also occurred with routing enabled and automatic failover disabled. It was triggered when switching to opencode go's deepseek-v4-flash without selecting 1M; other models did not reproduce it. A comment also reported that baseurl forwarding did not work when routing was enabled and an API that did not require routing was selected, but no confirmed fix was provided.

   Edit ~/.claude.json. Move PROXY_MANAGED from customApiKeyResponses.rejected to customApiKeyResponses.approved.
