Cause

This was reported on Windows with CC Switch 3.17.0. Codex context compression failed and may have sent an excessively large context upstream, causing a crash. The report also noted that hook trust and plugin states were not persisted, but the issue provides no evidence that these symptoms share the same root cause as the compression failure.
