"Failed to upload "image.png"" screenshot upload failed - cc-switch
Quick fix
This is not a cc-switch error. GitHub failed to upload the screenshot. Re-upload it or paste the error as text so maintainers can see it.
Symptom
Section titled “Symptom”<!-- Failed to upload "image.png" -->When submitting an issue or comment on GitHub, clicking submit before an image finishes uploading—or an interruption caused by a proxy, network instability, or an oversized file—leaves only an HTML comment placeholder such as `<!-- Failed to upload "image.png" -->` in the body. The image itself never reaches the server. Because HTML comments are hidden after rendering, others see only a blank area: you think you posted a screenshot, but maintainers cannot see anything.
Multiple issues in this cluster had this problem. In some, the only error evidence was a screenshot that failed to upload. For example, in #6046, a maintainer explicitly said the screenshot had not uploaded and they had never been able to see the specific error; the issue was eventually closed after more than 60 days without activity. In others, some screenshots uploaded, but the critical error screenshot was missing, as in #2901, #116, #4617, and #4668. This placeholder is unrelated to cc-switch. It is not an error produced by the application, but evidence of a failed upload on the GitHub web interface. The appearance of cc-switch versions 3.12.3 - 3.19.1 and various platforms in these issues is purely coincidental.
After pasting an image into the GitHub editor, wait until the thumbnail and user-attachments link are fully generated before submitting. Do not submit while it shows "Uploading..." or only a comment placeholder.
Check the rendered content after submitting. If `<!-- Failed to upload "image.png" -->` appears in the body or the image area is blank, the upload failed. Edit the content and upload it again, or retry on another network by disabling or changing the proxy.
If the screenshot still will not upload, use text instead. Copy the exact error from the terminal or Claude Code / Codex into the body inside a code block, and include the cc-switch version, operating system, and base URL with the Key redacted. This is the information the maintainer requested in #6046.
If the issue was closed as stale because the error was not visible, follow the maintainer's advice in #6046: reopen the issue and include the exact error text, base URL, and CC Switch version so troubleshooting can continue.

- Is `<!-- Failed to upload "image.png" -->` in the body an error from cc-switch?
- No. It is a placeholder comment left when an image upload fails on the GitHub web interface and is unrelated to cc-switch. It means the screenshot never reached the server, so nobody, including maintainers, can see it.
- What happens if I submit an issue before the screenshot uploads?
- Maintainers cannot see the error information and cannot investigate it. In #6046, a maintainer explicitly said the screenshot had not uploaded and they had never been able to see the specific error. The issue was eventually marked stale and closed after more than 60 days without activity.
- Is there another way to describe the problem if screenshot uploads keep failing?
- Yes. Paste the exact terminal error into the body as text, and include the cc-switch version, operating system, and the relevant provider base URL with the API Key redacted. Text errors are usually easier to search and diagnose than screenshots.
