Cause

When switching from a third-party provider back to an official Codex account to use a personal quota, the Log out / Log in steps were unclear. The README approach cited in the original post is to first add the official provider among the preset providers, then complete one sign-out and sign-in. You can then switch between official and third-party providers.

The steps in the comment apply only to the Visual Studio Code Codex extension: open settings from the upper-right corner of the extension, sign out of the personal account, then sign in again. The issue is closed, but the original post has no follow-up confirmation from the requester.
